Case Management

Clients entering Level 2.5 or 2.1 treatment at RTI receive case management services. Case Management involves assessing client needs, evaluating risks, treatment planning, implementing treatment, transition planning, transition plan implementation, and monitoring client progress.

Case Management is a collaborative process between the client and their counselor to assure that appropriate care is provided during treatment and post-discharge. Case Management services include but are not limited to a comprehensive biopsychosocial drug and alcohol assessment, collaborative treatment planning, collaborative transition planning, provision of resource referrals as well as guidance and assistance with arranging aftercare.
